How do I order my own personal quilt?
First, you print out the Order Form below, submit payment (either by personal check or VENMO), and send me the completed order form and your T-shirts, sweatshirts and/or special fabric that you want used in the quilt. By special fabric I mean the material used to make the core panels NOT the material for the sashes and borders - we provide that in the color of your choosing. IMPORTANT: Please contact us via email PRIOR to sending in order so we know to expect it.
How long until I get my order?
We will ship your new machine quilted personal quilt approximately 6 weeks after receiving your T-shirts, sweatshirts and/or special fabric.
What if we have a question with your order?
If we have any questions, we will contact you via email or phone.
To print out the order form, click Download File below. Once you have printed and filled out the form, email MyPersonalQuilt@gmail.com to let us know that you are sending in your t-shirts, sweatshirts or special fabric. Once that is done, box up the order form, your check made payable to Jeryl Weiler and your t-shirts, sweatshirts and/or special fabric and send to the address on the order form.
